Nurse IBD advice clinic for Crohn’s Disease and Ulcerative Colitis

Our nurse-led advice clinic is dedicated exclusively to supporting patients with a confirmed diagnosis of Crohn’s disease or Ulcerative Colitis. We provide expert guidance, personalised care, and ongoing support to help you manage your condition confidently.

What the IBD clinic offers:  

We provide comprehensive support tailored to your needs, including: 

  • Chronic disease management 

  • Medication advice, diet and lifestyle guidance 

  • Monitoring and clinical intervention 

  • Individualised care plans 

  • Regular follow-up appointments and flare management, including personalised flare plans. 

We offer care for patients receiving biologic medications, including education on biologic therapies, ongoing support and monitoring and medication management guidance.  

We are committed to helping you understand and manage your condition through one-to-one sessions and planned group presentation evenings.  

Appointments: 

Standard consultations are 20 minutes with our IBD Nurse Specialist. Patient experience and continuous service improvement are at the heart of everything we do, and we aim to provide compassionate, responsive, and personalised care at every stage of your journey. 

Eligibility & diagnosis: 

You must have a confirmed diagnosis of Crohn’s disease or Ulcerative Colitis; diagnosis is established following a colonoscopy and confirmed by a doctor. Once diagnosed, you can access the clinic at any stage of your condition, and the service is available regardless of how long you have been diagnosed for.

When to seek medical advice:

While the clinic provides ongoing support, patients should contact their GP if they experience abdominal pain, blood in their stool or concerning stool or blood test results.  

If you feel acutely unwell or suspect a severe flare, please attend your GP or A&E immediately.
