Patient Services

Anybody can be seen at the Queens Road Medical Practice as long as they are not registered with another Practice in the island.

Visitors to the island are welcome to be seen at the Practice, but only Guernsey residents can register as a patient with one of our Doctors.

If you would like to register as a patient at the Queens Road Medical Practice, please telephone 01481 724184 to book a ‘registration appointment’ with either a Nurse or a Doctor.

Please complete and return the below form to Queens Road Medical Practice with a copy of your photographic ID. The New Patient Administrator will then be in contact to arrange a registration appointment.

Please note that you are not registered as a patient until you have attended your registration appointment and been officially accepted by the Practice.

In the event that a patient informs us that they want to leave the Practice we will do all we can to help ensure that their transfer to their new Doctor is as smooth as possible.

If the patient is moving to another Guernsey practice, we will need to receive a request from the new Doctor before we can forward them a copy of the patient’s medical records.

If the patient is moving off island, records can be sent without a request from the new Doctor.

Should the need arise, patients’ medical records may be shared with other professionals involved in patient care.

Sharing of medical information will be under the following guidelines as published in the Data Protection (Bailiwick of Guernsey) Law, 2001.

The processing is necessary for medical purposes and is undertaken by:

  1. a health professional; or
  2. a person who in the circumstances owes a duty of confidentiality which is the equivalent to that which would arise if that person were a health professional

“Medical purposes” includes the purposes of preventative medicine, medical diagnosis, medical research, the protection of vulnerable individuals, the provision of care and treatment, the management of healthcare services and as other Legislation requires.

Medical records may be accessed upon request in accordance with Guernsey Data Protection Legislation.  You may view the original records at the Practice with a member of staff (or your Doctor if you have medically related questions).

If you believe that your medical records contain incorrect information, please do not hesitate to discuss this at the earliest opportunity with your Doctor or a member of the Practice management team who will be happy to correct any factual errors.

If you have any queries please do not hesitate to contact a member of the management team or your Doctor.

If you would like to access your data, please fill out the Subject Access Request form below:
